Foundations Health provides outpatient mental health and addiction treatment services. Our outpatient center fosters a culture of healing and community to help with your process of health and wellness. This outpatient program provides medication-assisted treatment (MAT) in addition to counseling services. Medication Assisted Treatment providing Methadone and Buprenorphine products for individuals diagnosed with Opioid Use Disorder, Moderate or Severe with wraparound services for mental health disorders.
